Whether long-term care, Hospice and end of life, respite care, recovery or personal care in your home for seniors, pediatric or anyone else who may be needing assistance with their daily routine.

 

What Services We Offer +

Anchor caregivers provide services of traditional in-home care agencies, but we also go beyond that to offer meal planning and assist with organizing therapy such as physical therapy, speech therapy occupational therapy, and even activity therapy.

 

Where we serve +

Anchor caregivers are distributed throughout West Michigan. We cover Muskegon, Grand Haven, Holland and Saugatuck / Douglas. Also Grand Rapids, East Grand Rapids, Ada, Cascade, Kentwood, Hudsonville and Grandville to name just a few!

In-Home Health Services

Care is available on an hourly basis with a three-hour minimum, 24 hours a day, seven days a week including holidays.
Home Health Aides are trained to provide non-medical services including the following:

 

Personal Aid & Companionship

• Mindful Assistance For Alzheimer’s, Dementia and Memory Care Clients
• Exceptional Assistance with Activities of Daily Living Including Eating, Bathing, Dressing, Toileting
• Transferring, Continence and Other Services as Requested.
• Planning and Conducting Meaningful Therapeutic Activity
• Light Housekeeping Services and Laundering Assistance
• Presentation of High Quality Meals: Planning, Shopping, Food Preparation & Service and Clean Up
• Companionship: Meaningful and Loving Relational Engagement, Pairing of Staff for Best Client Rapport
and Maintaining Consistency of Caregivers to Build Closeness, Trust and Confidence.

 

Health Care Management

• Care Coordination and Client Advocacy
• On-going Care Evaluation and Assessment
• Attending Appointments
• Aid in Implementation of Care Directives
• Medication Management and Reminders
• Documentation of Activity and Observations

 

Spiritual Care

• Reading Scripture
• Leading or Participating in Devotionals
• Participating in Church Services
• Listening and Conversation

 

Transitional Care

After a hospital stay or surgical procedures such as knee or hip replacement. Transitional care provides for optimal recovery.

 

End-of-Life Care

Many Anchor caregivers have Hospice experience. We routinely partner with local Hospice chapters to augment their end-of-life care.

 

Respite Care

Designed to give the primary caregiver rest and relief. Family members may need time to recover from exhaustion, or may need help to provide care for a time while the primary caregiver is away or unavailable.

 

Sunrise Anchor Care Service

3 morning hours, seven days per week. Services include daily living activities to prepare for the day, such as assistance with showering, hygiene, dressing and breakfast service. Other morning services may be required such as medication assistance, meal prep for the remainder of the day, and housekeeping.


 

Sunset Anchor Care Service

3 evening/night hours, seven days per week. Services include daily living activities to prepare for bed, including evening meal preparation and service, evening medication assistance, and housekeeping.

 

Sunrise & Sunset Anchor Care Service

All services included in the Sunrise & Sunset, 3 morning and 3 evening hours of the client’s choice equaling 6 hours per day.

 

Overnight Anchor Care

Caregivers provide overnight care, offering peace of mind and rest for family members who may be primary caregivers. Caregivers remain wakeful, alert and working. When client does not require personal care, caregivers engage in other services, such as quiet housekeeping, laundry, meal preparation and more; 8 hours.

 

Overnight Service + Sunrise or Sunset Anchor-Care

Caregiver will provide overnight care for client and peace of mind for primary caregiver. Housekeeping services available at this time in additional to a sunrise or sunset service of client’s choice; 10 hour minimum.

 

Daylight + Sunrise or Sunset Anchor Care

Client choice of sunrise or sunset services + daylight hours in the home for meal prep, companionship, housekeeping, pet care, errands or other client preferences; 8 hour minimum..

 

24/7 Anchor Care

Complete 24/7 care and companionship. Personal care, housekeeping, errands, meal prep, or other client preferences; All services in four care classifications;

24 hours.

 

Hourly Anchor Care

This bill rate is for Ala-carte requests, not originally planned into the service agreement. 3 hour minimum.

 

Covid-19 Anchor Care

For positive Covid-19 clients. The Covid-19 rate will remain in effect for 14 days after exposure. After 14 days, subject to re-evaluation.

 Holidays are billed at 1.5 the hourly rate, there are six per year. Client home to provide all supplies, such as masks, gloves and gowns if required.
